This permit to work relates to any work carried out at elevated place as described
in DSK-0512.

☐ Are devices used for work such as Boatswain chair strong enough for safe

☐ Are tools and instruments for work secured with lifting rope or put into lifting

☐ Are Working time and nature of work informed to the parties concerned in
case that work is carried out in the vicinity of funnel, whistle radar and
aerial?
