abilityofourpublishertocondensetheeditorialmaterialintoasinglevolume.
Thesinglevolume,furthermore,wassupportedbyelectronicmaterialandwasin
fullcolor,withuniformlysuperbillustrationsprovidedbyGemmaPrice.
Now,inthisfourthedition,thetextbookcantrulyclaimtohaveachieved
globalprominence.Thisistruenotonlyintermsofgeography,butalsothe
disciplinesforwhichitaimstocater.Thus,JimTweddelljoinstheteamasthe
firstsurgicaleditor,whileKathyMussattobringsmuch-needednursing
excellence.GilWernovskyhasnowtakenovertheroleofeditorialdirector.The
overallteamisnowbasedfirmlyintheUnitedStates.Anothersignificant
addition,nonetheless,isthattheteamisjoinedbyKrishnaKumar.KKbringsto
thetabletheamazingadvancesbeingmadeintheIndiansubcontinent.The
writtenandillustrativecomponentsofthebook,furthermore,arenowsupported
byvideomaterial,collatedandorganizedbyJustinTretter,withwhomIhave
beenparticularlypleasedtocollaborateextensivelyoverthelastfewyears.We
havebeensorrytoloseDanPennyfromoureditorialteambutpleasedthathe
wasstillabletocontributeinupdatingthechaptersheproducedandeditedfor
thethirdedition.
Whenwestartedourendeavour,inthebuild-uptothepublicationofthefirst
editionin1987,wechosetobasethebookonthesignificanceofanatomy.We
havebeenabletoretainthisfocusthroughoutalleditions.Itisamazingtomeas
tohow,withthepassageoftime,theanatomicfeaturesaredemonstratedwith
ever-increasingaccuracyduringlife.Thisisdemonstratedevenmoresointhis
edition.Theimagesproducedbyvirtualdissectionofcomputedtomographic
datasets,atleasttomyeyes,nowshowthemorphologicfeaturesatleastaswell
asweareablewiththeheartsinourhands.Inthefirstthreeeditions,theother
editorsindulgedmyownidiosyncracies,permittingmetoexpungeabbreviations
andprovideauniformstyle.Ifancythatmaywellhavechangedinthisedition.
Forme,personally,ithasbeenahugeprivilegetobeabletopursuemy
“hobby”asmycareer.Ihavebeenequallyprivilegedtomeetandcollaborate
withwonderfulpeopleinthefieldofpediatriccardiology,notleastthosewho
havefulfilledtheeditorialrolesinproducingthepreviouseditionsofthisbook.I
amtrulygratified,therefore,thatthecurrentteamofeditorsandthepublisher
chosetotitlethisfourtheditioninmyname.Nothingcouldserveasbetter
recognitionofallIhopedtoachievewhenstartingmycareerasacardiac
anatomist,supportedsostronglyinitiallybyElliotShinebourneandthenby
FergusMacartneyandMichaelTynan.Itisnowmyhopethatthebookwill
continuetogofromstrengthtostrengthinfutureeditions.Itisingoodeditorial
hands.Inclosingmyforeword,Imustacknowledgethehugecontributionsmade

Itishardtoimagine,asBobAndersonpointsoutinhisforeword,thatitis36
yearssincethefirsteditionofthistextbookwaspublished.Bob'scontributions
toourfieldarelegendary,ofcourse,andhiscontributionstothisbooksimilarly
so.Thefourthadditionisnoexception,andwhileitistruethatnoteverylinehas
beensubjectedtotheAndersonian“redpen”treatment(yes,someabbreviations
andsplitinfinitiveshavecreptin),therootsofthebookcontinuetolieinhis
detailedexpositionofsegmentalanalysisofthecongenitallymalformedheart
anditsanatomicunderpinnings.Itisafittingtributethatthishasbecome
Anderson'stextbook.
While,sadly,publicationofthefirsteditiondoesnotpredatethebirthofany
ofthecurrenteditors,itdoespredateourinvolvementinthefield.Thecircleof
professionallife,nonetheless,hasseenususethebookastrainees,contributeto
thebookaschapterauthors,andnowoverseethedevelopmentofthebookasit
lookstowardthefuture.Somethingshavenotchanged,andevenBobhas
managedtokeepthenomenclatureofsomethingsconstant!Buttherehave,of
course,beenextraordinaryclinicaladvancesduringthelifetimeofthistome.
Backin1987,whenthebookestablisheditselfasoneoftheleadingresourcesin
ourfield,thearterialswitchprocedurefortranspositionwasjustbecoming
established,theNorwoodprocedureforhypoplasticleftheartsyndromewas
rarelyperformed,andwhilecross-sectionalechocardiographywasfully
establishedasthediagnosticmodalityofchoice,itstransesophagealmodality
wasattheverybeginningsofitsevolution,andcolorflowDopplerwasafew
yearsaway.Furthermore,whilethefirstMRIimageoftheheartwasobtainedin
thatyear,itwouldbeadecadeormorebeforeitbecamepartofourroutine
clinicalpractice.
Necessarily,therefore,justasourfieldhasdeveloped,sohasourbook.The
deliveryofoutstandingclinicalcareincongenitalheartdiseasehasalwaysrelied
onthemultidisciplinaryteamapproach.Theeditorialteamnowreflectsthat
moreappropriately.Thestructureofthebookalsoreflectsourchangingworld.
