Skeletally immature children and adolescents are at higher risk for
epiphyseal or physeal fractures around the knee; therefore,
radiographs are recommended for evaluation of possible fracture after
a knee injury (Fig. 111.40 ).
Adolescents and teenagers more commonly sustain ligamentous
injuries that may be associated with avulsion fractures.
With distal femoral epiphyseal fractures, adequate analgesia is
important to decrease muscle spasm, protecting the physis from further
damage.
Distal femoral epiphyseal fractures can be complicated by compartment
syndrome or direct compression to vascular and nerve structures
around the knee.
Popliteal vessel or peroneal nerve injury can occur with distal femoral
epiphyseal or proximal tibial epiphyseal fractures.
