Abstract
Thischapterdiscussesavarietyofarteriovenouscommunications,with
specialemphasisonthosethatresultincardiovascularconsequences,such
asvolumeand/orpressureoverloadofaspecificvascularbed,and
cyanosis.Wedistinguishbetweenacquiredneoplasmscomposedof
endothelialcells(hemangiomas)fromprimitiveerrorsofvascular
development(arteriovenousmalformationsandarteriovenousfistulas).

Therehasbeendramaticprogressinthefieldofvascularanomaliesinrecent
years,includingimprovedunderstandingoftheclassificationoftheselesions,
theirnaturalhistory,andoptimalmanagementstrategies.1Thissectiondiscusses
onlyvascularanomaliesthathavethepotentialtoposecardiovascular
ramifications,eitherduetoabundantarteriovenousshuntingpresentingwith
congestiveheartfailure(CHF)orpulmonaryhypertensionorduetoright-to-left
shunting(attheintracardiacorintrapulmonarylevel)presentingwithcyanosis.
Becauseofthehighlyvariedandimpreciseterminologiesusedinthepastto
describeahostofvascularanomalies,olderpublicationsaredifficulttorelyon
foraccuratediagnosesandarepredominantlyusefultoprovidehistoricalcontext
regardingtheearlydescriptionsofsuchconditions.Oneoftheearliestreportsof
“congenitalhemangioendothelioma”(atermthatshouldnolongerbeused
becauseitdoesnotconveywithclaritytheprecisenatureofthelesionaccording
