Abstract
Inordertounderstandtheanatomyofthecongenitallymalformedheart,it
isfirstnecessarytoappreciateallthedetailsofnormalanatomy,including
thearrangementsofthecoronaryarteriesandtheconductiontissues.We
beginbyemphasizingtheneedtodescribetherelationshipsofthecardiac
componentsastheyareseenwiththeheartnormallylocatedwithinthe
thorax.Fortoolong,anatomistsandpathologistshavedescribedtheheart
asseensubsequenttoitsremovalfromthebody,andpositionedonitsapex.
This“valentine”positioncurrentlyunderscoresthetermsusedtodescribe
structureswithintheheart,whichintheeraofthree-dimensional
reconstructionsincreasinglyusedinclinicalsituations,doesnothingbut
createconfusion.Inthischapter,therefore,wedescribethecomponentsof
theheartasseeninattitudinallyappropriatelocation,illustratingthedetails
usingimagescreatedfrommultidetectorrowcomputedtomographic
interrogationofadultsundergoinginvestigationforcoronaryarterial
disease.Wethendescribethefeaturesofthevariouschambersthat
underscoretheapproachtothecongenitalcardiacmalformationsthatare
subsequentlydescribedintheremainderofthebook.

withtheheartnormallypositionedwithinthechest,theattitudinallyappropriate
